Allergy and Anaphylaxis Emergency Plan
Please complete the above form if your child has a life-threatening allergy and may require the administration of Epinephrine and/or Benadryl while at school. The form is an Emergency Action Plan that will be followed if your child is exposed to an allergen.
Medication will not be accepted in the nurse’s office without the proper completion of the plan. All medication must be in its original dispensing container. Epinephrine pens must have a prescription label which matches the healthcare provider's order.
Please check the expiration date of the medication prior to dropping it off at the nurse's office.
