Kindergartens and New Students

Kindergarten Orientation Presentation

All students entering Cherry Hill Public Schools are required by the district policy and NJ state law to have the following documents:

1. A valid immunization record. This must include the following: 

DTaP (Diphtheria, Tetanus, Pertussis) 

Any 5 doses or any 4 doses with the last dose given after the 4th birthday.

IPV (Polio) 

Any 4 doses or any 3 doses with the last given after the 4th birthday.

MMR (Measles, Mumps, Rubella)

Two doses; the first dose given after the 1st birthday.

Varicella (Chicken Pox) 

One dose after the 1st birthday (two doses are suggested).

HBV (Hepatitis B) 

Three doses total.

Exemptions can be made for a student who provides either:

  • A written statement signed by a parent or guardian explaining how the administration of immunizing agents conflicts with the pupil's exercise of bonafide religious tenets or practices. General philosophical or moral objection to immunizations shall not be sufficient for exemption.

OR

  • A written statement from a physician stating that a specific immunization is medically contraindicated.

 

2. A health physical that has been completed and signed by a physician within 365 days of the pupil's registration with the school. Please click here for a universal physical form.

3. An updated health history record that has been completed and signed by a parent or legal guardian.

4. An emergency contact form with valid points of contact that can be reached should your child encounter an emergency situation while at school.
