Medications
Please complete this form if you would like the nurse to administer any medication to your child during school hours (exceptions are asthma medication and epinephrine pens; please see the "Asthma" and "Allergies" tabs).
A new form must be turned in at the beginning of each school year.
I will administer the medication to your child exactly according to what is written on the Medication Authorization Form and will notify you if any changes are needed.
Please be aware:
- All medication must be transported to and from school by an adult. Please do not send students with medication in their backpacks, whether over the counter or by prescription, as it is against school policy.
- During the school day, parents/legal guardians are permitted to administer medication to their child in the nurse's office. Students cannot take medication in the classroom.
- The medication must be contained within the original dispensing container. For prescription medication, the dispensing medication container must display a valid pharmacy label. The written request from the parent/guardian (also referred to as the Medication Authorization Form) shall give permission for such administration and relieve the Board of Education and its employees from liability for administration of medication, and in the case of administration of acetaminophen or ibuprofen, shall further indicate the specific condition for which administration is requested.
- The written request (also referred to as the Medication Authorization Form) from the prescribing physician shall include:
- The name and purpose of the medication
- The dosage of the medication (including the instructions for administration)
- The name of the healthcare provider
- The date
